An investigation has been launched into the death of a passenger who fell from a London Underground station platform and was hit by a train.
The incident took place at Ickenham station, which is on the Uxbridge branch of both the Piccadilly and Metropolitan lines, on the evening of March 28.
After falling onto the track at around 10.30pm, the passenger was hit by a Tube train before they were discovered by staff at the Zone 6 station.
The passenger — whose identity remains unknown — sustained fatal injuries as a result of the accident in west London.
The Rail Accident Investigation Branch, a government agency that independently investigates rail accidents in Britain, launched an probe into the death on Wednesday.
The RAIB said that it would seek to identify the sequence of events that led to the accident last month.
It will consider the actions of those involved at the time, the management of staff, and the arrangements in place to manage the risk of accidents.
The probe will be independent of any investigation by the railway industry or by the industry regulator, the Office of Rail and Road.
Lilli Matson, Transport for London’s chief health, safety and environment officer, said: “Our thoughts are with the family and friends of the person who sadly died at Ickenham Tube station last month.
"We are supporting the RAIB with its investigation and are conducting our own internal inquiry to understand the causes and any actions that can prevent such tragic incidents in future.

Ickenham station opened as a halt for the Metropolitan Railway in 1905 following lobbying by Ickenham Parish Council.
It was initially a small stop intended to serve day-trippers from central London to rural areas.
The platforms were later extended in 1922, and the station was rebuilt into its current form in the 1970s.
It served approximately 850,000 passengers in 2023.
Six passengers have been killed in “passenger-train interface” (PTI)” incidents on the Tube in the four years to the end of 2023/24, according to TfL.
